Schedule a Telephone Consultation

  • APPOINTMENT: Submit the form below, and Marguerite will schedule you for a telephone consultation.

    Schedule an initial consultation ($50 for an hour consult).
    Schedule a follow-up consultation ($30 for a half hour consult).

  • PHONE CONSULTATION: Marguerite will review your answers on the questionnaire, create a treatment plan specific to your needs, answer any questions you may have, and email you a written step-by-step treatment plan. Changes in your prescription medication may be recommended, but any prescription changes will have to be prescribed by your personal physician.

  • PRICING: The price for an initial one hour consultation is $50. If a consultation requires more than one hour to complete, a second appointment will be needed, unless the following hour is open. Follow-up consultations are $30. Missed consultation appointments will be billed $30, unless cancelled by email at least 15 minutes prior to the scheduled appointment.
